Description:Notre Dame At Home- HospiceClinical team LeadFull Time (40 hours)
Notre Dame Hospice Care provides comfort, compassion, companionship, and support to patients nearing the end of life and their family members when a treatment or cure for an underlying disease is no longer available. Services are delivered by a dedicated team of health care professionals who focus on addressing physical, mental, social and spiritual needs to improve quality of life.
The Clinical Team Lead provides strategic and operational oversight of the hospice interdisciplinary team, including Nursing, Social Work, Spiritual Care, Home Health Aides, Music Therapy, and Massage Therapy. This role ensures that all care delivered aligns with established standards of practice, state and federal regulations, and organizational protocols. The Team Lead fosters clinical excellence, timely and accurate documentation, and a collaborative team environment. As a key leader, the RN Clinical Team Lead ensures that each hospice patient and family receives a comprehensive, interdisciplinary array of services, and that the team consistently operates with a unified, patient-centered approach.
Requirements:· Graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
· Active and unrestricted Registered Nurse (RN) license in the Commonwealth of Massachusetts.
· Compliance with all applicable state and federal regulatory requirements.
· Minimum of five (5) years of hospice nursing experience required.
· Certified Hospice and Palliative Nurse (CHPN) credential preferred.
· Prior experience in hospice leadership or management strongly preferred.
· Working knowledge of hospice reimbursement guidelines and regulatory standards preferred.
· Demonstrated ability to work effectively within an interdisciplinary team.
· Proven leadership skills with the ability to foster collaboration, guide team performance, and maintain professional standards.
· Ability to manage patient care in a compassionate, efficient, and cost-effective manner.
· Strong commitment to the philosophy and principles of hospice care.
· Strong clinical assessment and symptom management skills.
· Excellent interpersonal skills with the ability to build rapport with staff, patients, families, and external partners.
· Valid driver’s license and access to reliable transportation.
· Willingness to participate in continuing education to maintain licensure and stay current with best practices.
· Exceptional verbal and written communication skills.
