Job Title: Member Care Coordinator

Contract Duration: 2 Years

Location: Albuquerque, NM

Schedule: Full-time, Field-Based (3–4 days/week travel)

Summary:

The Member Care Coordinator conducts home visits and health assessments, educates members on their care plans, and coordinates non-clinical services to support overall case management. This role works closely with clinical teams to ensure members receive appropriate support and resources.

Key Responsibilities:

• Perform in-home health assessments and outreach
• Educate members on health plans and available services
• Coordinate appointments, transportation, and follow-ups
• Document member interactions and update records
• Support case managers with administrative tasks
• Travel locally based on member needs (3–4 days/week)

Requirements:

• Bachelor’s in Social Work, Psychology, or related field
• OR LVN/LPN with 1 year managed care experience OR RN
• OR 3+ years in care coordination or managed care
• Proficiency in Microsoft Office (Excel & typing test required)
• Valid driver’s license and reliable transportation
• Strong communication and customer service skills
