The Applicant must hold a bachelor degree in social sciences
Job description:
1.Be a member of the Multidisciplinary team to evaluate social work needs of each patient
2.Develop a care plan for each patient
3.Promote peer group relationships through problem-centered discussion and task-centered groups.
4.Provide in-home assessment of prospective patient
5.Provide group sessions in the program
6.Provide participant, caregiver and family counseling as needed
7. Some more will be explained during the interview
Work hours: Monday - Friday, 8:00 am - 2:30 pm
