Job Requirements
- ASCP certification required
- Primary Source Verification of professional license and/or certification within 30 days of start date and updated upon expiration
- MA Conditional Licenses not accepted
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break
- MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break
- MGB Resume including past 7 years of work experience with city & state
- 2 recent references, minimum 1 Supervisor or Above, in Skillset
- MGB Skills Checklist required for submission
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
- MGB Education Verification of highest level education achieved
- MGB SSN Trace listing addresses/alias for past 7 years
- MGB National Sex Offender Check within 30 days of start and updated annually
- MGB Exclusion Checks within 30 days of start and annually
Schedule Information
- Contract length is 13 weeks
Additional Details
- Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines or proof of immunity via blood test
- Hepatitis B vaccination record or proof of immunity or declination (strongly preferred)
- Influenza vaccine required during flu season Sept 15th - April 30th with no exemptions or waivers
- TB skin test screening within 3 months of start date and updated if >6 month break or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break
- Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
- Tdap vaccine within 10 years and updated upon expiration (preferred)
- Respirator Medical Clearance Attestation required if fit test is required for skill
- Positive PPD requires negative chest x-ray documentation
- MGB NWH Self-Study Orientation & Attestation available in Document Library
