Job Title: Full Service Partnership (FSP) Care Manager – Adults Program

Location: Los Angeles, CA 90006

Schedule: Monday – Friday, 8:30 AM – 5:00 PM

Compensation: $25 – $28/hour (DOE)

Driving Role: Yes – Field-based (Mileage Reimbursed)

Position Overview:

We are seeking a dedicated and compassionate FSP Care Manager to join our Adult Full Service Partnership (FSP) program. This field-based position supports adults experiencing moderate to severe mental health challenges through coordinated, community-based care. The ideal candidate is a strong team player, highly organized, and experienced in working with high-complexity mental health populations.

Key Responsibilities:

• Maintain a caseload of 20–25 adult clients, providing intensive field-based support.
• Conduct resource linkage, rehabilitation counseling, and care coordination with medical and behavioral health providers.
• Collaborate as part of a multidisciplinary care team, including social workers, case managers, and psychiatric staff.
• Document all services in the Avatar electronic health record (EHR) system
• Participate in regular staff and consultation meetings.
• Provide on-call support approximately once every 6–8 weeks (additional compensation provided for calls received).
• Travel to client locations, which may include shelters or transitional housing (no encampment outreach required).

Qualifications:

• Bachelor’s degree in Psychology, Social Work, or a related behavioral health field.
• Experience working with adults with severe and persistent mental illness.
• Valid driver’s license and access to a personal vehicle (mileage reimbursement provided).
• Comfortable working independently in the field and navigating community-based settings.
