MDS Coordinator - Large Senior Living Community

We are seeking an outstanding Registered Nurse (RN) to join our team at a large senior living community at Willow Tree Care Center Skilled Nursing & Rehab, Delta, CO.

Job Description

The MDS Coordinator is responsible for the overall coordination of the facility's Medicare, Resident Assessment Instrument (MDS), and care planning process.

Responsibilities

• Assess and evaluate the quality of care being given to long-term care residents
• Perform the initial interview and assess new residents
• Develop a care plan that has long-term and short-term goals for improvement
• Conduct meetings with multidisciplinary treatment teams, caretakers, and families
• Reach out to specialists within the community to get help in achieving care plan goals

Qualifications

• Graduate of an accredited nursing program
• Must have a valid, unrestricted RN license in the state
• Previous MDS experience preferred
• Previous experience in Long Term preferred, but not required
